Description

  • Provide telephone triage for hospice patients and their families after hours.
  • Assess patient conditions and determine the appropriate next steps for care.
  • Coordinate with on-call teams to support patient care and resource needs.
  • Document all calls, communications, and interventions accurately in EMR systems.
  • Maintain a calm, professional, and empathetic demeanor during urgent calls.
  • Support timely patient care coordination in a remote after-hours setting.

Requirements

  • Active multistate Licensed Practical Nurse (LPN) license.
  • Hospice, palliative, or end-of-life care experience is strongly preferred.
  • Comfort using technology and electronic medical records (EMR) for call documentation.
  • Ability to type documentation and notes in a fast-paced environment.
  • Ability to handle stress and multitask while receiving at least 5 calls per hour on weekdays and up to 8 per hour on weekends.
  • Strong communication and critical thinking skills.
  • Ability to work independently in a remote environment.
  • Must maintain consistent attendance, active communication, and reliable internet connectivity during all scheduled shifts.

Benefits

  • Base pay of $19 per hour with opportunities to increase up to $22 per hour within the first 6 months.
  • $2 shift differential for overnight shifts.
  • $2 shift differential for weekend shifts (Friday evening, Saturday, and Sunday).
  • 3 weeks of paid remote training.
  • PTO accrual for both part-time and full-time nurses.
  • 401(k) plan eligibility for part-time and full-time nurses.
