Description

  • Deliver coordinated, patient-centered care management by telephone and video to improve members’ health outcomes.
  • Create care plans with members and the care team and help members work toward their goals.
  • Guide members through complex medical conditions, treatment pathways, benefits, and the healthcare system.
  • Partner with members’ local providers to coordinate care across settings.
  • Provide compassionate longitudinal follow-up and build supportive relationships with members and families.
  • Support members and families through acute episodes such as hospitalizations and rehabilitation stays.
  • Coordinate resources that address members’ clinical and social needs holistically.
  • Gather clinical histories, answer questions at a patient level, and summarize findings clearly for colleagues and care planning.

Requirements

  • Bachelor of Science in Nursing (BSN).
  • Current CCM certification or eligibility to obtain it within 6 months of hire.
  • 5+ years of nursing experience.
  • 2+ years of care management experience.
  • Active Compact RN license in good standing and residence in a compact NLC state.
  • Willingness to obtain and maintain licensure in multiple states.
  • Availability to work Monday-Friday, 9:00am-6:00pm PST.
  • Preference for candidates based in MST or PST time zones.
  • Experience working remotely and using multiple computer and medical record systems.
  • Experience with patients across age ranges and comfort discussing multiple medical conditions.
  • Spanish-speaking ability is desirable.
  • Strong empathy, communication, and ability to write medical information in clear, patient-centered language.
  • Ability to follow security and HIPAA regulations.
