Description

  • Monitor high-risk pediatric patients up to 21 years old and adjust care plans based on follow-up needs.
  • Conduct biopsychosocial assessments to identify behavioral, social, emotional, and systemic needs.
  • Create, evaluate, and update patient and family care plans as goals and needs change.
  • Provide crisis intervention and stabilization support for patients and family members.
  • Deliver psychoeducation on mental health diagnoses, treatment adherence, and related topics.
  • Coordinate case management support across complex healthcare and social service systems.
  • Resource external services such as social service agencies and behavioral specialists.
  • Collaborate with pediatricians, nurses, care coordinators, school systems, specialists, and child welfare agencies.
  • Participate in interdisciplinary consultations to monitor patient progress.
  • Represent Imagine Pediatrics professionally to patients, families, providers, and the community.

Requirements

  • Master’s degree in social work or a related field.
  • LMSW required.
  • 3-5 years of post-graduate experience in healthcare social work or behavioral health case management.
  • Experience working with pediatric populations and family systems.
  • Proficiency in motivational interviewing, goal setting, intervention, and needs assessment.
  • Knowledge of crisis prevention and intervention in social work.
  • Experience providing telehealth services.
  • Knowledge of MS Office Suite and online platforms.
  • Strong understanding of behavioral health principles, trauma-informed care, and pediatric/adolescent mental health.
  • Ability to work remote, travel 10% for training/education, and work afternoons/evenings.
  • Bilingual Spanish strongly preferred.
