Description

  • Conduct monthly outreach calls to patients managing chronic conditions.
  • Support patients with care plan adherence and medication adherence.
  • Perform health risk assessments and monitor patient health data remotely.
  • Document clinical interactions accurately in the EHR system.
  • Serve as a primary point of contact for patient questions and care coordination support.
  • Assist with Remote Patient Monitoring (RPM) tasks.
  • Collaborate with the broader clinical care team to communicate patient updates and concerns.
  • Help improve patient engagement metrics and maintain quality-of-care standards.
  • Manage a patient panel and anticipate clinical needs before they become urgent.

Requirements

  • Active, unencumbered CMA or RMA certification.
  • Current Basic Life Support (BLS) certification.
  • Proven experience in a clinical or care coordination setting.
  • Strong proficiency with EHR systems and virtual documentation.
  • Excellent verbal and written communication skills.
  • Ability to explain complex health information clearly.
  • Dedicated HIPAA-compliant, distraction-free home office with high-speed internet.
  • Ability to work independently and manage a structured daily workflow without direct supervision.
  • Prior experience in Chronic Care Management (CCM) or Transitional Care Management is preferred.
  • Familiarity with community resources and social determinants of health is preferred.
  • Experience with quality metrics and CMS compliance standards is preferred.
  • Success in high-volume, fast-paced remote healthcare environments is preferred.
