Description

  • Accept transfers from the Patient Enrollment team and complete preliminary health assessments for newly enrolled patients.
  • Respond to patient questions and maintain open dialogue to understand patient needs.
  • Identify and coordinate community resources that support patient care.
  • Provide patient education and health literacy support for chronic condition management.
  • Assist with medication management, including identifying medication concerns, adherence issues, and refill coordination.
  • Coordinate timely delivery of services such as Home Health, DME, Home Infusion, and other patient needs.
  • Work primarily by phone in a fast-paced environment while adapting quickly to changing patient needs.
  • Document and track patient conversations and care needs with thorough real-time notes.

Requirements

  • Active Compact/Multi-State LPN or LVN license.
  • At least 3 years of direct patient-facing work experience.
  • Technical aptitude with Microsoft Office Suite.
  • Excellent written and verbal communication skills.
  • Must have active high-speed Wi-Fi.
  • Must have a home office or HIPAA-compliant workspace.
  • Experience with Epic (preferred).
  • Bilingual ability (preferred).
  • Additional single-state licensures (preferred).
  • Ability to sit for long periods and communicate clearly by telephone for extended periods.
