Description

  • Conduct compassionate telephonic advance care planning conversations with patients and families to identify healthcare goals and wishes.
  • Educate patients and families about medical interventions such as feeding tubes, mechanical ventilation, and CPR.
  • Document conversation details accurately within 48 hours to produce state-specific advance care planning documents.
  • Provide support through incoming patient calls, documentation review, and special projects as needed.
  • Maintain knowledge of state-specific advance care planning laws and regulations through training and 1:1s.
  • Navigate multiple systems while speaking with patients and using call center and documentation tools.
  • Support high-volume periods through voluntary overtime when needed.
  • Perform other assigned duties as needed.

Requirements

  • Certificate or diploma in Practical Nursing.
  • Current and unrestricted LPN license.
  • 2+ years of experience as an LPN or in a relevant role.
  • Demonstrated knowledge of serious medical conditions such as COPD, CHF, and ESRD and their progression.
  • Ability to adapt to shifting priorities and a fast-paced environment.
  • Familiarity with Google Suite, including Google Meets, Google Sheets, and Google Docs.
  • Ability to learn new call center, telephony, and patient documentation technology quickly.
  • Strong verbal and written communication skills with active listening and empathy.
  • Strong time management skills and attention to detail.
  • Familiarity with state-specific advance directives and Medical Power of Attorney regulations, as well as ACO and Medicare/CMS guidelines.
  • Experience in palliative care or hospice is preferred.
  • CHPN or CHPLN certification is preferred.
  • Inpatient hospital experience, including ICU or emergency medicine, is preferred.
  • Spanish-speaking experience is preferred.
