Description

  • Provide direct supervision of care coordinators’ day-to-day activities in Sonoma, Orange, and San Bernardino Counties.
  • Participate in patient outreach, care coordination, and provider communication as an individual contributor.
  • Coach, mentor, and support care coordinators to ensure competency and workflow compliance.
  • Coordinate with local leadership, care team members, Operations, and Learning & Development to execute cross-team workflows and projects.
  • Review data dashboards to monitor team performance, identify improvement opportunities, and drive goal achievement.
  • Help hire, train, deploy, and support local care coordinators as the market grows.
  • Foster partnerships with primary care providers through communications and meetings.
  • Build and maintain community partnerships through in-person or virtual engagement at community events.
  • Maintain knowledge of community programs and resources that support patient health care needs.
  • Promote an inclusive, collaborative team culture and represent Waymark’s values within the local team.

Requirements

  • Experience supervising care coordinators, patient navigators, customer service staff, outreach specialists, or similar roles.
  • Strong understanding of Medicaid and managed care organizations (MCOs), including barriers to access and quality for underserved populations.
  • Demonstrated ability to monitor and optimize metrics using Excel or similar data tools.
  • Experience collaborating and communicating effectively in a multidisciplinary team setting.
  • Experience with healthcare data and/or electronic health records.
  • Ability to travel locally within the market, up to 30%.
  • Must reside in Sonoma, Orange, or San Bernardino County.
  • Bachelor’s degree in health administration, health management, public health, business administration, or a related field (preferred).
  • Longtime resident of one of the three counties with knowledge of MCO and community resources (preferred).
  • Multilingual skills are a plus.
