Description

  • Conduct routine patient check-ins to monitor therapy progress, identify concerns, and support adherence to treatment plans.
  • Complete initial outreach to confirm therapy start and assess early treatment experience.
  • Coordinate patient appointments, follow-up visits, and clinical monitoring schedules.
  • Support pharmacists by gathering patient information, preparing charts or notes, and managing task queues.
  • Track required labs and monitoring parameters, ensure completion, and communicate status to the care team.
  • Prepare and route prescription-related requests and medication renewals to the pharmacist for review.
  • Maintain clear, timely documentation in required systems to support continuity of care.
  • Organize patient lists, call queues, and clinical follow-up tasks to improve workflow efficiency.
  • Maintain confidentiality and adhere to clinic standards, policies, and professional expectations.
  • Participate in training, team meetings, and process improvement initiatives.

Requirements

  • State registration as a pharmacy technician is required.
  • Nationally certified as a CPhT or equivalent is strongly preferred but not required.
  • Must commit to obtaining any additional required licensure within a mutually agreed timeframe if needed.
  • 1+ years of pharmacy technician experience is required.
  • 5-8 years of pharmacy technician experience is preferred.
  • Experience working with physicians, nurses, and other healthcare professionals is a plus, preferably in a medical clinic.
  • High school diploma or GED is required.
  • Strong interpersonal communication skills are required.
  • Strong phone skills are required.
  • Highly proficient in Epic and Microsoft Office, especially Excel and Word.
  • Able to quickly learn other programs and extract relevant information.
  • Strong organizational skills are essential.

Benefits

  • Hourly compensation of $28.00-$36.00 per hour.
  • $5,000 sign-on bonus.
  • Remote to start, with potential for a future hybrid arrangement depending on business needs.
