Description

  • Coordinate patient care and services to ensure a smooth flow of referrals and new admissions into clinic locations.
  • Conduct phone interviews with patients to gather demographics, assess needs, confirm eligibility, and schedule initial visits.
  • Review medical records, insurance coverage, and related documentation to support eligibility and care planning.
  • Collaborate with nurses, nurse practitioners, physicians, and other healthcare professionals to support patient-centered care.
  • Explain available services to patients and families and set clear expectations throughout the care process.
  • Arrange and schedule home visits and help adjust care plans based on patient progress.
  • Maintain accurate records in electronic medical systems to support billing and regulatory compliance.
  • Build professional relationships with referring agencies and community partners to improve patient access to care.
  • Participate in quality improvement initiatives, ongoing education sessions, and adherence to professional standards.
  • Represent NIVA Health with discretion, diplomacy, tact, and strict patient confidentiality.

Requirements

  • Experience in clerical, intake, administrative, or U.S.-based healthcare settings.
  • Previous experience in Home Health or Hospice is a plus.
  • Comfort using medical software and Microsoft Office tools, including Word and Excel.
  • Strong written and verbal communication skills with patients, caregivers, and staff.
  • Strong organizational skills with the ability to multitask in a fast-paced remote environment.
  • Critical thinking and problem-solving skills with a patient-first mindset.
  • Ability to work full-time on a remote schedule in the U.S. East Coast timezone.
  • Associate degree or higher is preferred.
  • Experience with electronic medical records (EMR) systems is preferred.
  • Experience working with U.S.-based healthcare insurance, Medicare, and Medicaid regulations is preferred.
