Description

  • Design, implement, and continuously refine program workflows, care pathways, and operating models to improve patient outcomes and program performance.
  • Partner with Clinical Product to shape patient journeys, protocols, and program evolution.
  • Serve as the central point of coordination across patients, partner PCPs, internal clinical and operational teams, and leadership.
  • Own program execution while ensuring alignment with goals, service commitments, and regulatory requirements.
  • Coordinate program logistics, resourcing, scalability, and new client onboarding.
  • Provide direct patient care focused on fracture risk identification, prevention, and longitudinal bone health management.
  • Educate patients on osteopenia and osteoporosis, treatment options, risks, and follow-up care.
  • Conduct risk assessments, coordinate screenings such as DXA scans, and communicate results and next steps.
  • Initiate and manage treatment plans in alignment with clinical protocols and collaborate with partner PCPs on orders and coordinated care.
  • Lead and support a Patient Coordinator, including hiring, onboarding, training, coaching, and workflow management.

Requirements

  • Active, unrestricted Nurse Practitioner (APRN) license in Florida, or ability to obtain it before start.
  • National board certification as a Nurse Practitioner (AANP, ANCC, or equivalent).
  • Eligibility for prescriptive authority in Florida.
  • Registered as an Autonomous APRN in Florida, or able to obtain autonomous registration upon hire.
  • 3+ years of clinical experience, including at least 2 years of clinical case management experience.
  • Experience coordinating care across providers, including collaboration with primary care physicians and external partners.
  • Strong clinical judgment with the ability to assess risk, initiate treatment plans, and guide longitudinal care.
  • Comfort working in a structured, protocol-driven environment while applying sound clinical decision-making.
  • Exceptional communication and influence skills with the ability to motivate patients and build strong PCP partnerships.
  • Highly organized, able to manage multiple workflows, priorities, and stakeholders simultaneously.
  • Prior people leadership experience, including training, coaching, or managing team members.
  • Preferred: experience in care management, care navigation, or population health models.
  • Preferred: experience building or scaling a clinical program, service line, or care pathway.
  • Preferred: experience managing patients with osteopenia, osteoporosis, or chronic disease conditions.
  • Preferred: familiarity with bone health management, including DXA interpretation and osteoporosis treatment protocols.
  • Preferred: experience in a value-based care or risk-bearing environment.
  • Preferred: experience in telehealth or remote-first care delivery models.
  • Preferred: experience with clinical protocols, SOP development, or quality improvement initiatives.
