Description

  • Provide telephone triage support for hospice patients and their families during after-hours coverage.
  • Assess patient conditions and determine appropriate next steps based on client protocols.
  • Coordinate care and resources with on-call teams to ensure timely interventions.
  • Document all calls, communications, and interventions accurately in the EMR.
  • Maintain a calm, professional, and empathetic demeanor while handling urgent calls.
  • Support patient-centered care for hospice and palliative service needs in a remote setting.

Requirements

  • Active multistate Registered Nurse (RN) license.
  • Hospice, palliative, or end-of-life care experience is strongly preferred.
  • Comfort using technology and electronic medical records (EMR) for call documentation.
  • Ability to type documentation and notes in a fast-paced environment.
  • Ability to handle stress and multitask, including a minimum of 5 calls per hour on weekdays and up to 8 calls per hour on weekends.
  • Strong communication and critical thinking skills.
  • Ability to work independently in a remote environment.
  • Consistent attendance, active communication, and reliable internet connectivity during all scheduled shifts.
  • Comfort supporting after-hours care coordination in a high-volume triage role.

Benefits

  • Base pay of $25 per hour after training, with potential to earn up to $28 per hour within the first 6 months.
  • 3 weeks of paid remote training.
  • $2 shift differential for overnight shifts.
  • $2 shift differential for weekend shifts.
  • Paid time off accrual for both part-time and full-time nurses.
