Description

  • Provide telephone triage for hospice patients and families after hours.
  • Assess patient conditions and determine appropriate next steps.
  • Coordinate care and resources with on-call teams and client care teams.
  • Document all calls, communications, and interventions accurately in the EMR.
  • Maintain a calm, professional, compassionate demeanor while handling urgent calls.
  • Support timely patient care coordination by responding to incoming calls in a fast-paced environment.
  • Work independently while maintaining consistent communication during scheduled shifts.

Requirements

  • Active multistate Registered Nurse (RN) license.
  • Hospice, palliative, or end-of-life care experience strongly preferred.
  • Comfort with technology and electronic medical records (EMR) for call documentation.
  • Ability to type notes and documentation accurately in a fast-paced environment.
  • Ability to handle stress and multitask while managing a high call volume, including at least 5 calls per hour on weekdays and up to 8 per hour on weekends.
  • Strong communication and critical thinking skills.
  • Ability to work independently in a remote environment.
  • Consistent attendance, active communication, and reliable internet connectivity are required for all scheduled shifts.

Benefits

  • Base pay of $25 per hour once trained to the originally assigned team.
  • Potential to earn up to $28 per hour within the first 6 months of hire.
  • $2 overnight shift differential and $2 weekend shift differential.
  • 3 weeks of paid remote training.
