Description

  • Provide telephone triage support for hospice patients and families after hours.
  • Assess patient conditions and determine appropriate next steps for care.
  • Coordinate with on-call teams to support care delivery and resource needs.
  • Document calls, communications, and interventions accurately in the EMR.
  • Maintain a calm, professional, and empathetic demeanor during urgent calls.
  • Support timely interventions that help coordinate patient care during non-core hours.
  • Work independently while serving as an extension of the client care team.
  • Manage a fast-paced call volume and handle multiple patient needs efficiently.

Requirements

  • Active multistate Registered Nurse (RN) license.
  • Hospice, palliative, or end-of-life care experience strongly preferred.
  • Comfort using technology and electronic medical records (EMR) for documentation.
  • Ability to type documentation and notes in a fast-paced environment.
  • Ability to handle stress and multitask in a high-call-volume setting.
  • Strong communication and critical thinking skills.
  • Ability to work independently in a remote environment.
  • Consistent attendance, active communication, and reliable internet connectivity during scheduled shifts.
  • Comfort handling a minimum of 5 calls per hour on weekdays and up to 8 calls per hour on weekends.

  • Base pay of $25 per hour after training, with potential to earn up to $28 per hour within the first 6 months.
