Description

  • Drive evidence-based care pathways for patients and proactively prevent complications.
  • Identify and remove bottlenecks in referrals and authorization workflows so care is not delayed.
  • Curate and coordinate community, clinical, and financial resources for each case.
  • Serve as the single point of alignment between doctors, insurers, and families on care objectives.
  • Audit caseloads and prioritize high-risk patients for proactive intervention.
  • Reduce time-to-care by navigating insurance barriers and administrative hurdles.
  • Track patient progress, outcomes, and utilization trends using digital tools and data.
  • Contribute to operational playbooks and improve case management processes.
  • Communicate effectively with clinical, insurance, and family stakeholders throughout the care journey.

Requirements

  • Professional degree such as RN, BSN, LCSW, or MSW.
  • Experience in acute care, home health, or complex care coordination.
  • Proven experience managing high-acuity cases and making critical decisions under pressure.
  • Tech-native mindset with the ability to use EHRs, data, and digital tools to manage workflow.
  • Demonstrated success preventing hospital readmissions or securing denied services for patients.
  • Ability to handle a 30+ case load with strong personal organization.
  • Primary internet connection of at least 15 Mbps.
  • Backup internet connection of at least 10 Mbps that can support work during a power outage.
  • Desktop or laptop with an Intel Core i5 (8th generation or newer), Intel Core i3 (10th generation or newer), AMD Ryzen 5, or equivalent processor, and at least 8 GB RAM.
  • Backup device meeting or exceeding Intel Core i3 performance and functional during power interruptions.
  • Functioning webcam, noise-canceling USB headset, quiet dedicated home office space, and smartphone for communication and verification.
