Description

  • Serve as the primary point of contact for new patients via phone and email during the enrollment process.
  • Guide patients through initial enrollment steps and explain program requirements clearly and empathetically.
  • Proactively support patients in completing onboarding tasks and track progress to reduce drop-off.
  • Register and onboard a high volume of new patients by phone.
  • Collect and verify patient insurance details, ID cards, and other required information.
  • Confirm patient eligibility and active insurance coverage for Form Health programs.
  • Set up patient charts in the EMR system and prepare them for the next phase of care.
  • Perform outreach to encourage patients to complete outstanding onboarding steps.
  • Share patient feedback and operational insights with internal teams to improve tools and workflows.

Requirements

  • At least 2 years of inbound phone experience in a fast-paced healthcare setting, patient enrollment, or high-volume customer service role.
  • Proven ability to manage a high volume of tasks independently while meeting enrollment targets.
  • Excellent verbal and written communication skills with the ability to communicate clearly and empathetically.
  • Ability to remain calm and professional during challenging calls or technical issues.
  • Proficient computer skills and ability to adapt quickly to evolving technologies.
  • Experience with platforms such as Slack, Google Workspace, EMR systems like Elation, or custom enrollment tools.
  • An empathetic, patient approach when working with diverse patient populations.
  • A proactive mindset with adaptability and creative problem-solving abilities.
