Description

  • Conduct outbound outreach to prospective patients to explain the program, assess eligibility, and guide enrollment from first contact through confirmation.
  • Document enrollment activity accurately, follow up on interested prospects, and escalate potential eligibility barriers for review.
  • Complete pre-visit outreach for newly enrolled patients to confirm appointment details, review logistics, and ensure they feel prepared.
  • Walk patients through visit technology on iOS and Android, troubleshoot basic connectivity or device issues, and escalate unresolved problems.
  • Verify insurance eligibility and confirm consent documentation before the initial care navigation appointment.
  • Monitor scheduling status throughout the program, coordinate appointment confirmations and reminders, and intervene when visits are delayed.
  • Follow up with no-shows and non-responders to re-engage patients and recover missed appointments within the appropriate window.
  • Maintain accurate, real-time documentation of patient contacts, eligibility status, scheduling activity, opt-outs, and escalations.
  • Escalate any clinical question, symptom, safety concern, or unresolved authorization issue to the appropriate clinical contact without improvising.

Requirements

  • 2+ years of experience as a Medical Assistant, EMT/Paramedic, Medical Technician, Patient Care Technician, or in an equivalent clinical support role.
  • Prior remote or telehealth patient engagement experience.
  • Experience handling a high volume of patient contacts with professionalism and warmth.
  • Background in front desk work, patient outreach, care coordination, telehealth support, or scheduling alongside clinical duties.
  • Warm, clear communication skills, especially with older adults and patients who are anxious, reluctant, or struggling with technology.
  • Strong organizational instincts and comfort working in a fast-moving, ambiguous environment.
  • Ability to recognize when clinical issues need escalation and act without hesitation.
  • High school diploma or equivalent; formal clinical training or certification strongly preferred.
  • Comfort using care management platforms, EHRs, and other digital tools.
  • Located in and authorized to work in the US without sponsorship.
  • Experience in high-volume primary care, specialty care, or care management scheduling.
  • Familiarity with Medicare-enrolled or older adult populations, Medicare coverage, and prior authorization processes.
  • Experience with care management or referral management platforms.
  • HIPAA training or demonstrated understanding of patient privacy requirements.
  • Bilingual ability, such as Spanish, depending on the target population.
