Description

  • Manage a patient census and work to close gaps in clinical and non-clinical care needs.
  • Identify and coordinate community resources that support patient care.
  • Provide patient education and health literacy support for chronic condition management.
  • Perform medication management, including reconciliation, adherence support, refill coordination, and identifying potential concerns.
  • Coordinate timely delivery of services such as home health, durable medical equipment, home infusion, and other patient needs.
  • Address patient questions and maintain open communication to understand care needs.
  • Assist with referrals and appointment scheduling.

Requirements

  • Active Compact/Multi-State LPN or LVN license.
  • At least 3 years of direct patient-facing work experience.
  • Experience identifying resources and coordinating needs for chronic care management patients.
  • Technical aptitude and proficiency with Microsoft Office Suite.
  • Excellent written and verbal communication skills.
  • Ability to work from a HIPAA-compliant home office with active high-speed Wi-Fi.
  • Ability to sit for long periods and communicate clearly by telephone for extended periods.
  • Epic experience is preferred.
  • Bilingual ability is preferred.
  • Additional single-state licensures are preferred.
