Description

  • Conduct outreach to patients virtually and in person, including appointment-related outreach and initial intake.
  • Support patient registration and enrollment through community engagement and outbound calls.
  • Provide care navigation for an assigned panel of patients, including appointment logistics, prescription support, lab support, referral coordination, and follow-up.
  • Assess social determinants of health needs and connect patients to community resources and SDOH programs.
  • Collaborate with a multidisciplinary care team to contribute to care plans, triage requests, and resolve complex patient needs.
  • Document patient interactions and care team communications across multiple systems and tools.
  • Participate in community events, group education sessions, and relationship-building with community organizations and key contacts.
  • Deliver culturally and linguistically appropriate health education and information.
  • Support federal and state program enrollment, scheduling, referrals, and continuity of care.
  • Use motivational interviewing to support individualized goal setting and social needs assessments.

Requirements

  • Fluency in both English and Spanish, verbal and written.
  • High school diploma or GED.
  • California Community Health Worker certification, or 2,000+ hours of CHW work in the past three years with certification required within 18 months of hire.
  • 1-3 years of healthcare experience or healthcare navigation in the community.
  • 2-5 years of community work, advocacy, engagement, or organizing experience.
  • Previous experience in health promotion, project coordination, social research, or administration.
  • Familiarity with Google Workspace (preferred).
  • Experience documenting in an EHR (preferred).
  • Training in motivational interviewing (preferred).
  • Knowledge of Latino customs and cultural norms, strong ties to the local Latino community, and familiarity with local resources supporting SDOH needs are preferred.
  • Comfort using multiple computer applications simultaneously and willingness to learn new technologies and frameworks.
  • Ability to travel to other communities, attend in-person appointments and events, and have reliable transportation.
  • Must be authorized to work in the United States.
  • Must comply with Zócalo Health’s COVID-19 vaccination requirement, subject to applicable law and reasonable accommodation.
