Description

  • Execute person-centered care plans for members with serious, high-risk illnesses and help revise disease-specific plans as needs change.
  • Act as a clinical care bridge between patients and billing practitioners by supporting care coordination, provider scheduling, rescheduling, and post-discharge follow-up.
  • Conduct social determinants of health assessments and develop action plans to address unmet needs such as food insecurity, housing instability, and other barriers to care.
  • Help patients and caregivers navigate the California healthcare system, build self-advocacy skills, and coordinate transportation or telehealth access.
  • Provide motivational interviewing, emotional support, and tailored education to help members manage chronic illness and improve treatment adherence.
  • Support patient communication templates, workflow automation, and technical troubleshooting in partnership with the tech team.
  • Update patient charts and maintain accurate records in the Case Management System, including required time logs and activity documentation.
  • Refer members to community-based resources and maintain partnerships with local California organizations that provide legal aid, food assistance, and other support services.
  • Drive to public places and member homes within a 50-mile radius in California to conduct home visits and in-person member interactions.
  • Work independently in a remote team environment while coordinating closely with clinicians and clinical operations staff.

Requirements

  • 3+ years of relevant community outreach, facilitation, or healthcare experience.
  • Experience navigating high-risk chronic conditions or complex care management is a major plus.
  • Strong computer skills and the ability to use web-based and app-based systems with ease.
  • Deep understanding of the socio-economic and public health challenges facing disenfranchised populations in California.
  • Excellent written and verbal communication skills with an outgoing personality and the ability to motivate and influence others.
  • High school diploma or GED required; college degree preferred.
  • Valid driver’s license, reliable transportation, and a flexible schedule for occasional work outside regular business hours.
  • Ability to complete and maintain all CMS-mandated training and state-applicable requirements, including interpersonal building, service coordination, and professional ethical conduct competencies.
  • Commitment to a drug-free workplace and willingness to complete pre-employment substance abuse testing and background checks.
  • Ability to safely drive within a 50-mile radius in California, carry up to 30 pounds of supplies, and stand or walk for the duration of home visits or member interactions.
