Description

  • Execute person-centered care plans for members with serious, high-risk illnesses.
  • Monitor and help revise disease-specific care plans, including frequent medication or treatment changes.
  • Act as a clinical care bridge between the patient and the billing practitioner.
  • Coordinate care by managing provider availability, rescheduling, and post-discharge support.
  • Conduct person-centered assessments to identify social and cultural factors affecting care.
  • Develop action plans to address unmet social needs such as food insecurity and housing instability.
  • Help members and caregivers navigate the California healthcare system and advocate for their needs.
  • Provide support with transportation, telehealth access, and communication with specialists.
  • Support communication templates, workflow setup, technical troubleshooting, and chart updates in the Case Management System.
  • Refer members to community-based resources and maintain local California partnerships for supportive services.
  • Use motivational interviewing, education, and emotional support to improve adherence and help families and caregivers manage chronic illness.
  • Maintain detailed documentation of member interactions, time spent, and activities performed for Medicare-compliant services.

Requirements

  • 3+ years of relevant community outreach, facilitation, or healthcare experience.
  • Experience navigating high-risk chronic conditions or complex care management is a major plus.
  • Strong computer skills and ability to use web-based and app-based systems with ease.
  • Deep understanding of the socio-economic and public health challenges facing disenfranchised populations in California.
  • Excellent written and verbal communication skills with the ability to motivate and influence others.
  • High school diploma or GED required; college degree preferred.
  • Valid driver’s license and reliable transportation are required.
  • Flexible schedule for occasional work outside regular business hours.
  • Willingness to complete and maintain all CMS-mandated training and state-applicable requirements.
  • Must be committed to a drug-free workplace and able to pass pre-employment substance abuse testing and background checks.
