Description

  • Support the mental and physical health care of assigned patients in coordination with medical providers and other mental health professionals.
  • Screen and assess patients for mental health and substance use disorders and support engagement in follow-up care.
  • Educate patients and caregivers about mental health and substance use disorders and available treatment options.
  • Monitor clinical symptoms and treatment outcomes, document progress, and adjust care plans as needed.
  • Deliver evidence-based behavioral interventions such as motivational interviewing, behavioral activation, and problem-solving treatment.
  • Collaborate with psychiatric consultants to develop and adjust treatment plans for patients who are not improving as expected.
  • Facilitate referrals to external resources such as social services, vocational rehabilitation, and specialty care.
  • Maintain accurate documentation in Synapticure’s EHR and communicate clearly with the care team.
  • Develop relapse prevention plans with patients who are meeting treatment goals.
  • Build trust-based relationships with patients, caregivers, and partners to support continuity of care.

Requirements

  • Active RN or LCSW licensure.
  • Minimum of 2 years of experience in direct case management or care management.
  • Background in ALS, Parkinson’s disease, or other neurodegenerative or chronic disease care.
  • Strong communication and interpersonal skills.
  • Familiarity with insurance processes and medical billing/coding.
  • Experience reviewing medical records and working within clinical trials.
  • Ability to thrive in a startup or rapidly growing organization.
  • Knowledge of current trends in neurodegenerative diseases, terminal illnesses, targeted therapies, and clinical trials.
  • Passion for working with vulnerable populations and providing patient-centered care.
  • Remote work from home with travel to Chicago, IL or other meetings as needed.
