Description

  • Provide a full suite of custom medication management services to hospital patients as an off-site member of the clinic team.
  • Manage prior authorizations, complete benefit investigations, and act as an agent of the provider to resolve insurance-related issues.
  • Assist patients with medication onboarding, financial assistance assessments and securing financial assistance when available.
  • Conduct outbound therapy/medication adherence check-ups, secure prescription renewals, and support patient refill/renewal processes.
  • Educate prospective and current patients on how the hospital partner pharmacy serves them and improves overall care.
  • Document patient information and interactions in the hospital EMR, Therigy, and Shields proprietary tools.
  • Identify medications requiring special storage/handling and determine necessary supplies (e.g., sharps containers, needles, swabs).
  • Answer, resolve, and triage inbound inquiries and communicate directly with patients, on-site clinicians, payors, and pharmacies.
  • Input data, perform adjudication on test claims, prepare action plans for follow-up, and work to achieve operational objectives.
  • Participate in process improvement initiatives, apply observations to increase productivity and quality, and protect confidential information.

Requirements

  • State-registered pharmacy technician required.
  • National certification as a CPhT or equivalent strongly preferred; must commit to obtaining any additional licensure required within an agreed timeframe.
  • 2–3 years of experience required as a Pharmacy Technician; 3–5 years preferred.
  • High school diploma or GED required.
  • Strong interpersonal and phone communication skills; comfortable proactively engaging patients telephonically and contacting clinicians, payors, and pharmacies.
  • Highly proficient in Microsoft Office (particularly Excel and Word) and able to quickly learn proprietary systems and tools like Therigy.
  • Strong organizational skills, attention to detail, high integrity, accountability, and dependability.
  • Ability to sit and/or stand and focus at a workstation for extended periods; discretion when handling highly sensitive and private data.
  • Experience or knowledge in disease states such as transplant, hepatitis C, infectious disease (including HIV), diabetes, MS, or oncology is a plus.
  • Willingness to engage in ongoing education, maintain professional networks, and participate in professional organizations.

Benefits

  • Salary range $55,000–$70,000 plus bonus pursuant to the terms of any applicable bonus plan.
