Description

  • Provide comprehensive off-site medication management services to hospital patients, including onboarding and ongoing adherence support.
  • Serve as an off-site member of the clinic team handling prior authorizations, benefit investigations, and payer communications as an agent of the provider.
  • Resolve insurance-related issues and complete prior authorizations and financial assistance investigations to secure patient access to medications.
  • Perform patient outreach for therapy/medication adherence check-ups, refill/renewal support, and prescription renewals.
  • Document patient interactions and medication information in the hospital EMR, Therigy, and Shields’ proprietary tools.
  • Educate prospective pharmacy patients on hospital pharmacy services and support uptake of pharmacy programs.
  • Identify medications requiring special storage/handling and determine necessary supplies (e.g., refrigerated meds, sharps containers).
  • Answer, triage, and resolve inbound inquiries from patients, clinicians, payors, and pharmacies.
  • Input operational data, adjudicate test claims, prepare follow-up action plans, and participate in process improvement initiatives to meet operational objectives.
  • Protect patient confidentiality and apply observations to improve productivity, quality, and customer-service standards.

Requirements

  • State registration as a Pharmacy Technician required; national CPhT certification strongly preferred (or equivalent).
  • 2–3 years of experience required as a Pharmacy Technician; 3–5 years preferred.
  • High school diploma or GED required.
  • Willingness to obtain any additional required licensure within a mutually agreed timeframe; continued employment contingent on meeting licensure requirements.
  • Strong interpersonal and phone communication skills with comfort proactively engaging patients telephonically and contacting on-site clinicians, payors, and pharmacies.
  • High proficiency in Microsoft Office (particularly Excel and Word) and ability to quickly learn proprietary systems (e.g., Therigy) and hospital EMR.
  • Strong organizational skills, attention to detail, high integrity, accountability, and dependability.
  • Ability to sit and/or stand focused at a workstation for extended periods, maintain discretion handling sensitive data, and work collaboratively as part of a team.
  • Experience or knowledge in specialty disease states is a plus (transplant, hepatitis C, infectious disease/HIV, diabetes, MS, oncology).
