Description

  • Provide personalized counseling and psychosocial support to patients to improve symptoms, quality of life, and clinical outcomes.
  • Serve as a primary patient success partner by answering health questions, sharing educational resources, and supporting accountability.
  • Collaborate with interdisciplinary care teams through case consultation.
  • Conduct case management by assessing patient needs, coordinating care, and connecting patients with community resources.
  • Lead group sessions on psychoeducation, stress management, mindfulness, and meditation.
  • Conduct one-on-one visits to provide emotional support, coping strategies, and stress management techniques.
  • Document patient encounters and coordinate follow-up care to support positive health outcomes.

Requirements

  • Active LCSW license with licensure to practice in NY, FL, and MI; PA, LA, or OH also considered.
  • 2+ years of experience as an LCSW.
  • Bachelor’s and Master’s degrees required.
  • Experience communicating with patients asynchronously preferred.
  • Experience with GI conditions or weight management a plus.
  • Proficiency with telehealth platforms and electronic health record (EHR) systems.
  • Experience working with patients in a clinic setting preferred.
  • Strong interpersonal communication, problem-solving, and organizational skills.
  • Ability to thrive in a fast-paced, high-growth environment.
  • Strong personal computing skills; remote work experience preferred.
  • Passion for helping clients make behavior changes and manage chronic disease through functional medicine approaches.
