Description

  • Coordinate care across settings including inpatient, outpatient, post-acute, emergency department, and home care.
  • Educate members via phone and secure messaging about health conditions, new medications, and procedures.
  • Develop individualized care plans for members experiencing complex health challenges.
  • Proactively communicate with hospital case managers to assist with discharge planning and transitions of care.
  • Participate in process improvement initiatives and pilot programs to improve member care.
  • Collaborate with internal support teams to ensure continuity and quality of care for members.
  • Ensure compliance with applicable laws and regulations in care management activities.
  • Occasionally travel for team meetings and company events as required.

Requirements

  • Active, unrestricted RN license in the candidate’s state OR active compact multistate unrestricted RN license.
  • Ability to obtain additional state nursing licenses as needed to meet business requirements.
  • 2+ years of clinical experience in payer, hospital, outpatient, or community-based care management.
  • 1+ years of experience in care coordination and navigation.
  • Residence in one of the following states: Arizona, Florida, Georgia, or Texas (remote work location requirement).
  • Ability to travel occasionally for team meetings and company events.
  • Preferred: CCM (Certified Case Manager) certification.
  • Preferred: Bilingual in Spanish and/or Haitian Creole (reading, writing, speaking).
  • Preferred: BSN degree.
  • Preferred: Working knowledge of Milliman Guidelines.
