Description

  • Meet with mothers virtually and in person to provide education on maternal warning signs, preeclampsia, and postpartum recovery.
  • Teach birth positions and breathing techniques to help mothers feel prepared for delivery.
  • Build trusting relationships with members and provide support via calls or messaging between visits.
  • Work with a multidisciplinary care team, join daily huddles, and escalate clinical issues to specialists.
  • Maintain accurate documentation of care interactions in internal systems.
  • Connect members to community resources for needs such as food, transportation, and housing.
  • Share community and member insights with Nadia Care to help shape programs and services.
  • Attend administrative meetings and follow policies and procedures to support quality care.

Requirements

  • 3+ years of experience in community health, patient advocacy, or healthcare navigation as a Certified Perinatal Community Health Worker or Certified Medical Assistant.
  • Experience providing prenatal and postpartum health education.
  • Bilingual in English and Spanish.
  • Strong listening, communication, problem-solving, and cultural sensitivity skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Comfort working independently in the field while staying connected to a specialist team.
  • Valid driver's license, clean driving record, reliable transportation, and compliance with state inspection, registration, and insurance requirements.
  • Access to home internet and a phone with internet access.
  • Ability to learn and use AthenaOne, Welkin, and MacOs.
  • Start-up experience or adaptability in a fast-evolving environment is preferred.
  • Open to a flexible schedule including Monday-Friday shifts, occasional weekends, and a light on-call rotation.
