Description

  • Drive program engagement by communicating the value of Maven’s care coaching model and optimizing member opt-in rates.
  • Co-create SMART goals with members and track progress, adjust care plans, and course-correct as needed.
  • Manage a caseload of members through messaging, video appointments, and outreach.
  • Use clinical guidelines, motivational interviewing, and behavior change techniques to remove barriers and support healthier outcomes.
  • Assess member knowledge and barriers and provide personalized health education across different backgrounds and literacy levels.
  • Collaborate with providers to translate clinical recommendations into actionable next steps.
  • Monitor member health data and platform activity remotely to identify risks early and take appropriate action.
  • Coordinate care across Maven’s digital network, in-person provider networks, and external partners.
  • Participate in and help lead quality improvement efforts to strengthen care delivery and clinical outcomes.
  • Provide coverage and timely responses to ensure members receive appropriate care when needed.

Requirements

  • 1-2+ years of experience in health coaching or clinical work (such as nursing, behavioral health, or a related field).
  • Experience with patient education, motivational interviewing, and driving clinical outcomes.
  • Demonstrated experience working with the maternity population and/or people navigating family planning or fertility care.
  • Training and experience coaching individuals toward specific health goals such as medication management, nutrition, exercise, or blood pressure control.
  • Strong written and verbal communication skills with an empathetic, member-centered approach.
  • Experience working toward KPIs such as opt-in rates, follow-up adherence, and member satisfaction.
  • Ability to quickly learn and adapt to digital health tools and platforms in a fast-changing environment.
  • Comfort using AI tools in daily workflows to improve efficiency and outcomes.
  • Strong telehealth competency, including phone outreach and video calls, with reliable high-speed internet access.
  • Health coaching certification (NBC-HWC or ICF) preferred.
  • Startup or fast-paced environment experience preferred.
  • Experience coaching across video, text/chat, and phone channels preferred.
  • Comfort with Google Suite, CRM platforms, EHRs, and Zendesk preferred.
  • Active work authorization in the US required.
