Description

  • Deliver coordinated, family-centered care management by phone to improve pediatric behavioral health outcomes.
  • Develop personalized, goal-oriented care plans in collaboration with families and a multidisciplinary team.
  • Support members through behavioral health treatment plans, transitions of care, and acute behavioral health crises.
  • Help families navigate behavioral health systems, treatment options, benefits, and community-based resources.
  • Partner with pediatricians, behavioral health specialists, schools, and other care team members to coordinate aligned care.
  • Provide ongoing longitudinal support, listening, advocacy, and clinical guidance to families.
  • Coordinate resources that address clinical, behavioral, and social determinants of health affecting the child and family.

Requirements

  • Bachelor of Science in Nursing (BSN) required.
  • Must reside in a compact NLC state.
  • Active Compact RN license in good standing with the nursing board of their state.
  • Active California Nursing License required.
  • Willingness to become licensed in multiple states.
  • Current CCM certification preferred.
  • 5+ years of nursing experience.
  • 2+ years of experience working in a pediatric behavioral health setting.
  • Ability to work Monday-Friday, 9:00am-6:00pm PST.
  • Comfortable managing pediatric behavioral health conditions including anxiety, depression, ADHD, autism spectrum disorders, and trauma-related diagnoses.
  • Ability to manage volume while maintaining quality and a high standard of care.
  • Flexible and adaptable in a fast-paced, rapidly evolving care delivery model.
  • Familiarity with HIPAA and commitment to patient privacy and security.
  • Excellent verbal and written communication skills, including the ability to explain clinical concepts in parent-friendly language.
  • Technologically competent and able to work across multiple electronic systems.
  • Demonstrated success working in multidisciplinary teams to coordinate care.
  • Strong understanding of cultural and socioeconomic factors influencing pediatric behavioral health.
  • Exceptional attention to detail and documentation quality.
