Description

  • Conduct thorough assessments of medical, behavioral, and complex social needs.
  • Identify barriers to care, including food insecurity, unstable housing, transportation challenges, and insurance limitations.
  • Collaborate with families and multidisciplinary teams to establish individualized care goals.
  • Develop and maintain integrated care plans addressing medical, behavioral, and social needs.
  • Ensure timely follow-up on referrals, evaluations, and treatment recommendations.
  • Provide education to families on navigating local resources and agencies.
  • Offer emotional support and guidance to families facing challenging situations.
  • Advocate for resources, remove barriers, and connect families with available supports.
  • Build and maintain relationships with community organizations serving children with medical and behavioral health complexities.
  • Participate in ongoing interdisciplinary consultations to monitor patient progress.

Requirements

  • Master’s degree with major coursework in social work, community/social services, and/or case management.
  • LMSW strongly preferred.
  • License or certification preferred, such as LMSW, CCMC, or C-SWCM.
  • 2-3+ years of post-graduate experience in social service case management required.
  • Minimum 1 year of experience working with the pediatric population required.
  • Knowledge and application of motivational interviewing practices and/or techniques.
  • Comfort working with acute populations and familiarity with school and community resources highly preferred.
  • Experience providing telehealth services and engaging with families in a remote environment.
  • Bilingual Spanish strongly preferred.
  • Must be living in New Jersey.
  • Primarily remote role with 10% travel for offsite meetings, training, and educational purposes.
  • Availability to work 8:00am-5:00pm Monday-Friday, with 2 days set to 10:00am-7:00pm with supervisor.
