Description

  • Serve as a community-based, patient-facing resource for re-engaging patients and caregivers in assigned geographic areas.
  • Provide hands-on support with program re-enrollment, onboarding, and mobile app setup and use.
  • Conduct telephonic outreach to caregivers to support onboarding and re-engagement for patients who missed visits or have not used services.
  • Educate patients and caregivers about Imagine Pediatrics services and available community resources.
  • Maintain up-to-date knowledge of local resources and connect families to social services, healthcare providers, and SDOH supports.
  • Conduct community events and facility rounding to engage eligible patients and support ongoing participation.
  • Partner with behavioral health teams and local providers to support discharge planning and continuity of care.
  • Work with caregivers, families, facility staff, and leadership to identify barriers to care and improve access workflows.
  • Collaborate with the Patient Engagement Team, including Outreach Specialists and Registered Nurses, to support re-engagement goals.
  • Support non-clinical care coordination, including primary care connections, pharmacy access, discharge follow-up, and outbound engagement calls.

Requirements

  • Valid driver’s license.
  • Bachelor’s degree.
  • Ability to travel frequently, with the primary location remote in Southern Missouri and frequent travel throughout the area.
  • 3+ years of experience in a healthcare setting, preferably in a patient-facing hospital or facility role.
  • Experience working in a patient-facing hospital or facility role.
  • Experience working as part of an interdisciplinary team.
  • Ability to navigate ambiguity, think critically, take initiative, and stay organized.
  • Ability to manage multiple tech platforms.
  • National Community Health Worker Certification highly preferred.
  • Bilingual Spanish skills highly preferred.
  • Medical Interpreter Certification preferred.
