Description

  • Educate patients and caregivers about palliative care and the program by phone.
  • Schedule patients for palliative appointments and connect them with the appropriate care providers.
  • Respond to questions from patients, caregivers, and providers.
  • Review clinical files, close out completed palliative visits, and enter notes into the system.
  • Confirm that patient encounters are documented and that patient needs have been triaged.
  • Follow up with patients to support high-quality care delivery across Empassion’s provider network.
  • Review and close completed cases on a daily basis.
  • Balance time between clinical triage, patient calls, and administrative work.
  • Assist the supervisor in managing daily contact center and care coordination functions.

Requirements

  • Active LPN/LVN background with a minimum of 3+ years of experience.
  • Minimum of 3+ years reading and updating clinical notes.
  • At least 1+ year of relevant experience in a customer service call center.
  • Ability to receive calls from patients, caregivers, and providers.
  • Strong communication and written skills.
  • Detail-oriented with strong multitasking and strategic thinking abilities.
  • Comfort working with technology in a dynamic startup environment.
  • Secure Wi-Fi connectivity for fully remote work.
  • Spanish-speaking ability is a plus but not required.
  • Experience in deep healthcare, clinical, and contact center environments is preferred.
