Description

  • Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical care.
  • Identify and coordinate community resources for patients who would benefit from additional support.
  • Provide patient education and improve health literacy around chronic condition management.
  • Perform medication management, including concern identification, reconciliation, adherence support, and refill coordination.
  • Ensure timely delivery of patient services such as home health, DME, home infusion, and other critical needs.
  • Respond to patient questions and maintain open dialogue to understand needs and concerns.
  • Assist with managing referrals and scheduling appointments.

Requirements

  • Active Multi-State/Compact LPN or LVN license.
  • At least 3 years of direct patient-facing work experience.
  • Technical aptitude with Microsoft Office Suite.
  • Excellent written and verbal communication skills.
  • Experience with Epic is preferred.
  • Bilingual ability is preferred.
  • Additional single-state LPN licensures are preferred.
  • Must have active high-speed Wi-Fi.
  • Must have a home office or HIPAA-compliant workspace.
  • Position is sedentary and requires extended telephone communication and real-time note taking.
