Description

  • Provide care coordination and case management services for patients with acute and chronic conditions.
  • Serve as the primary point of contact for patients, families, and multidisciplinary clinical teams.
  • Support, educate, and advocate for patients to ensure their individual care needs are met.
  • Coordinate care plans, monitor patient progress, and encourage adherence to treatment regimens.
  • Use EMR or case-management software to document and track patient care.
  • Collaborate with healthcare providers and insurance companies as needed.
  • Uphold the organization’s core values of teamwork, resilience, open-mindedness, ownership, and positivity.

Requirements

  • Bachelor’s degree in Nursing, Social Work, Healthcare Management, or a related field preferred.
  • Comparable experience or an alternative degree may be considered.
  • Experience in customer service, care coordination, or case management is preferred.
  • Active RN license is encouraged for nursing professionals, but not required for all candidates.
  • Case Management Certification (CCM) is a plus.
  • Proficiency with EMR and/or case management software is beneficial.
  • Excellent communication, organization, and time-management skills are required.
  • Ability to work collaboratively within a multidisciplinary team.
  • Patient-first mindset rooted in empathy, integrity, and creative problem-solving.
