Description

  • Provide patient-facing medication therapy management services by phone, including comprehensive medication reviews, targeted medication reviews, adherence counseling, medication reconciliation, and clinical program education.
  • Review completed medication reviews and counseling performed by other virtual pharmacist users as assigned.
  • Evaluate, assess, and document patient data in Arine's software platform and other systems, and prepare written and oral reports on patient care and team progress.
  • Follow up with clinics, providers, pharmacies, and patients to resolve medication-related issues, including transitions of care.
  • Develop intervention strategies to improve medication utilization, support quality measures, and achieve established clinical goals.
  • Manage a consistent volume of patient, provider, and pharmacy interactions while maintaining productivity and service quality.
  • Meet individual and team performance metrics related to outreach, engagement, documentation quality, and clinical outcomes.
  • Identify workflow and platform improvement opportunities by participating in meetings, collaborating cross-functionally, submitting tickets, and using automation and technology to improve efficiency.
  • Provide daily sign-out and clear handoff communication to ensure continuity of care.
  • Coach and support Clinical Care team members and help orient new virtual pharmacist users to workflows and the platform.

Requirements

  • Doctor of Pharmacy (PharmD) from an accredited college or university.
  • Hawaii licensed pharmacist in good standing; multiple state licenses are a plus.
  • Knowledge of CMS quality measures, Star Ratings, HEDIS metrics, and Medicare Part C and Part D medication measures.
  • Knowledge of chronic disease states across multiple therapeutic areas, including management of patients with polypharmacy.
  • Knowledge of prescription and non-prescription drugs, including indications, efficacy, dosage, mechanisms of action, contraindications, and side effects.
  • Ability to review prescriptions for appropriateness considering patient age, sex, comorbidities, concurrent therapies, foods, and diagnostic procedures.
  • Ability to engage patients, promote adherence, and use motivational interviewing and counseling techniques.
  • Excellent written and oral communication skills, with the emotional intelligence to build rapport and navigate sensitive interactions.
  • Demonstrated reliability, accountability, follow-through, and high attention to detail in documentation.
  • Comfort working in a metrics-driven, fast-paced environment and ability to work independently with minimal oversight.
  • Strong computer skills, including Google Workspace and Mac environment proficiency, plus comfort with Slack, Five9, multiple browser-based systems, keyboard shortcuts, and basic troubleshooting.
  • Willingness and ability to obtain pharmacist licenses in other states as needed.
  • Ability to work 40 hours per week, Monday-Friday 6AM-6PM Pacific Time, with up to two rotating Saturday shifts per month.
  • Must be able to pass a background check.
  • Must live in and be eligible to work in the United States.
  • Preferred experience in ambulatory care, especially chronic disease management.
  • Preferred inpatient, hospital, or emergency department pharmacy experience with direct patient and provider interaction.
  • Preferred experience with geriatric populations or long-term care settings.
  • Preferred prior experience performing MTM services with various MTM platforms.
  • Preferred experience in a health plan, PBM, ACO, or managed care pharmacy setting.
  • Preferred experience in a high-volume healthcare or pharmacy environment, such as retail, community, or a metrics-based call center.
  • Preferred board certification in a specialty area.
  • Preferred PGY1 or PGY2 residency in relevant pharmacy specialties.
  • Preferred experience working in a remote or virtual healthcare environment.
  • Preferred experience with other electronic medical records.

  • Hourly pay rate of $57-$65 per hour.
