Description

  • Conduct targeted patient outreach through inbound and outbound calls to address care gaps and support timely care transitions.
  • Perform post-discharge follow-up, medication reconciliation, and care coordination for high-risk patients.
  • Identify clinical red flags and escalate concerns to help prevent unnecessary ER visits and readmissions.
  • Provide patient education using empathy, active listening, and persistence to improve engagement and reduce reluctance.
  • Document interventions, assessments, and medication reconciliation accurately across multiple EHRs while maintaining compliance.
  • Assess patient eligibility for concurrent programs and support enrollment or follow-up as needed.
  • Collaborate with primary care practices on Transitional Care Management, osteoporosis management, ED follow-ups, and other care gap initiatives.
  • Serve as a clinical resource across teams to support patient engagement strategies and organizational goals.
  • Adapt outreach and coordination approaches as needed and identify opportunities for process improvement.
  • Meet and sustain performance metrics while managing a high-acuity patient workload.

Requirements

  • Bachelor’s degree in Nursing (BSN).
  • Active, unrestricted Registered Nurse (RN) license, including a valid Compact RN license.
  • Minimum of 2 years of clinical experience in case management or discharge planning.
  • Experience with telephonic patient outreach and clinical assessments.
  • Demonstrated ability to prioritize and manage high-acuity cases.
  • Excellent communication and interpersonal skills.
  • Experience using electronic health records (EHRs) and population health tools.
  • Strong critical thinking and clinical judgment in complex, rapidly changing environments.
  • Adaptability and resilience in a fast-paced, evolving healthcare setting.
  • Ability to work independently and as part of an interdisciplinary team.
  • Experience working remotely is preferred.
  • Experience in value-based care is preferred.
  • Case Management certification (CCM or ACM) or care transitions certification is preferred.
  • Experience working with diverse and underserved populations is preferred.
